In-Person Dance Classes for Minors | His Birth House LLC

I, the undersigned parent or legal guardian ("Guardian") of the minor named below, hereby enroll my child in dance classes offered by His Birth House LLC ("Company"), operated by Richard Matthews. In consideration of my child's participation, I voluntarily agree to the following terms:

1. Assumption of Risk

I acknowledge that dance and physical activity carry inherent risks, including but not limited to muscle strains, sprains, falls, and other injuries. I voluntarily accept these risks on behalf of my child and confirm that my child is physically capable of participating in dance classes.

2. Release of Liability

To the fullest extent permitted by law, I hereby release, waive, and discharge His Birth House LLC, its instructors, staff, agents, and representatives from any and all claims, demands, injuries, damages, or losses arising out of or related to my child's participation in dance classes, including claims arising from the negligence of the Company or its representatives.

3. Medical Authorization

In the event of a medical emergency, I authorize His Birth House LLC and its staff to seek emergency medical treatment for my child if I cannot be reached immediately. I agree to be financially responsible for any costs associated with such treatment.

4. Code of Conduct

I agree that my child will conduct themselves respectfully toward all instructors and fellow participants. His Birth House LLC reserves the right to remove any participant from class without refund if behavior is deemed disruptive or harmful to others.

Photo & Video Consent

I agree to allow His Birth House to photograph and/or video record my child during classes and events, and to use such content for educational, promotional, and social media purposes.
